How do I handle a lost check claim?
To request re-issue of a lost check, follow the bank's instructions. Contact the bank for further instructions.
EPS Financial
- Log in to http://www2.epsfinancial.net/ (log in with your EPS user name and password).
- Locate and print the Lost Check Affidavit form.
- Complete the form and fax it to the EPS Financial at (484) 546-2997.
Republic Bank
Complete the LOST_CK form and fax it to the Republic Bank at (866) 300-3528. This form is available in the RB ERO handbook.
