  • Line F, "Date Business Commenced," on AZ screen 16 in the Partnership package
  • Line B1, "Date business began in AZ or date income was first derived from AZ source," on AZ screen B in the S Corporation package
  • Line G1, "Date business began in AZ or date income was first derived from AZ source," on AZ screen G in the Corporation package
